Need help with a formal letter of complaint about a manager

I am trying to write a letter of grievance about my manager, and have a meeting with the owners of the company that I work for.
I have specific instances of lack of knowledge of job skills. Personal retaliation and discussing poor performances with co workers, are also some the things that I need to write about.
Poor work ethic, and personal sexual innuendo are also involved. Passive aggressive management skills and zeroing in on one specific employee at a time, along with favoritism are also some of the complaints of the staff.
We have a very hostile work enviroment, coupled with disassociation in the workplace.
How should I start?
Should I list specific instances that I am personally aware of, even though it includes my co workers, and not always myself?

Oh good thought, should I have other co workers sign? I will have to ask them what they feel about that. They are considering a massive walkout.

My main advice to you is that you focus in on the two most important concerns. Discussing too many could make you look like a complainer and it also presents to the owners a situation that seems impossible to fix.
I would mention the sexual innuendos and discussing employee performance with other employees who have no business knowing about it.
Those are two serious offenses that need to be addressed immediately.
Keep your complaints brief, don’t give long emotional stories, and offer objective facts, times, and dates. Only include stories you have witnessed. Don’t give too many examples, don’t talk to much because the owner may want to ask questions.
